COVID-19 Health and Safety Protocols

The 2022 Forum Annual Conference will take place in the context of the ongoing Covid-19 pandemic, which has devastated communities across the country, especially communities of color. While we are eager to reconnect with our friends and colleagues in person, we remain vigilant and are planning our in-person conference in Seattle with the utmost care and with consideration.

One of our top priorities is to create a safe environment for our attendees and we ask that each of our attendees help us in creating spaces where ALL our attendees will feel comfortable and understand that risk tolerances vary from person to person. We are relying on everyone to do their part to keep themselves and everyone else safe during the conference. At this time, these are the protocols we have in place to help ensure the safety of all participants.

United Philanthropy Forum reserves the right to update our policies at any time. Please continue to refer back to this website to keep up to date. If there is a significant shift in policy, we will email all conference registrants directly.

Vaccination Requirement

All attendees are required to be fully vaccinated to attend the meeting in person and must submit proof of vaccination through our provider IHP. Individuals are defined as fully vaccinated fourteen (14) days after getting their final dose of a COVID-19 vaccine. Individuals are also required to have one booster shot, unless the original vaccination was completed within six months.

Only those individuals who have an approved reasonable accommodation for religious and medical will be able to attend the meeting in person. Accommodations will be made based on the honor system. Individuals who request and receive the exemption are required to submit a PCR test within 48 hours of the conference.

Proof of covid vaccination, booster, or exemption documentation will be submitted to our partner In House Physicians (IHP) for review and verification.

There is no extra app to download to your phone! All attendees should complete the verification form as soon as possible. IHP will send each attendee a PDF via email once verified - please save a copy of this PDF. IHP will share a list of verified attendees with the Forum, attendees should not need to provide anything additional at conference check-in.
